Turning strategy into action for healthier neighbourhoods

A neighbourhood plan is becoming an essential tool for helping local systems move from ambition to action. By bringing together population health data, capital priorities, asset data and service needs into one shared plan, it will provide clarity on what is needed locally and ensure that investment is directed where it can make the most difference.

As the government accelerates neighbourhood‑based models of care, including the creation of new neighbourhood health centres, having a coordinated plan becomes even more important. Without one, systems risk missing opportunities to integrate services, align funding and make better use of the buildings and assets they already have.
